Today the Centers for Disease Control and Prevention and Million Hearts® announced the 2014 Million Hearts Hypertension Control Challenge Champions. Thirty Champions from public and private health care practices and systems across the country are recognized for their success in helping patients control high blood pressure.
Keeping blood pressure under control prevents heart attacks and strokes and saves lives. That’s why Million Hearts® launched the Hypertension Control Challenge—to recognize health care teams and practices across the country for helping their patients keep blood pressure under control.
These Champion health care professionals and their teams achieved blood pressure control for at least 70% of their patients with hypertension through innovations in health information technology and electronic health records, patient communication, and health care team approaches. There is strong evidence that when providers focus on hypertension control, rapid and substantial improvements can be made, and heart attacks and strokes are prevented.
A full list of Champions and content you can use to promote the challenge and share with your stakeholders can be found on the Million Hearts® website. All materials have been cleared for public use and dissemination.
We hope you will join us in recognizing their hard work—many are located in your communities—and use this opportunity to share how evidence-based strategies and system changes can play an important role in controlling high blood pressure.
